**JLCA/JLCB - PHYSICAL EXAMINATION OF STUDENTS/IMMUNIZATION OF STUDENTS**

All children entering South Hadley Schools are required, unless otherwise exempted under state law, to have:
- a complete physical examination
- immunization against diphtheria, tetanus, whooping cough, polio, measles, mumps, rubella, varicella and hepatitis B
OR
- immunization and/or examination forms presented to the school.
A tuberculin test is recommended, but not required.
State law requires:
- physical examinations for all children at grade intervals determined by the Department of Public Health be conducted by the school physician when not completed by the child's own physician. We recommend that the child's own physician do this examination. Forms will be sent home in the fall for completion.
Vision and Hearing tests:
- are administered annually
- parents are notified of any problems and then urged to take their child to a specialist.
